Putting in place safety measures, but reacting only after an accident has happened is too late. Safety must be managed, incidents investigated and mitigations have to be found.

The sentence above remains a good wish on the wall, often titled "Safety Policy" - and daily work is done differently.

During my studies at the Technical University Graz (Postgraduate Master-program 'Traffic Accident Research - Aviation Safety') I tried to cover the idea of developing a simple SMS for a non commercial operator using complex aircraft (acc. EASA NCC) in my Master's Thesis.
